Please open and complete the forms to get started. Please complete the appropriate forms and bring to the first appointment or email back to susanlagasse@sbcglobal.net. Fill out the Patient Information Form and Outpatient Services Contract if you are an adult. If you are using your insurance, fill out the Insurance Authorization Form. Also, please fill out the Developmental Questionnaire if you are bringing in a child or teenager. Please review the HIPPA Privacy Notification Form and print it out for your records if you wish. You also may ask any questions about it when we meet in the office.  Please review and complete the telehealth form even if you do not expect to use telehealth. Covid conditions change quickly.
